The printed proxy form read, "Know all Men by these Presents, That I,  _____________, of Philadelphia, do make, constitute and appoint FRANCIS A. CUNNINGHAM or THOMAS J. BARRY, my true and lawful ATTORNEY for me and in my name to appear at the Annual Meeting of the CATHOLIC CLUB OF PHILADELPHIA, to be held on Saturday Evening, May 6th, 1916, and vote thereat as my representative as fully as I myself could if present in person, with power also an attorney or attorneys, under him for that purpose to make and substitute, and to do all lawful acts requisite for effecting the premises; hereby ratifying and confirming all that the said attorney or substitute or substitutes shall due therein by virture of these presents. . . ."
